Abstract
At SQM'94 a paper was presented about quality assurance in mission critical systems, Erskine 1. This contained a critique of the public failure of London Ambulance, when it became computerised, and offered a 'good practice' framework for managing computer projects. It became the beginning of a programme funded by Glasgow Caledonian University into why design methodologies so often failed to deliver added value in computer projects and to computer mediated jobs. An early stage of this research programme involved trying to profile the nature and extent of waste in mainly UK computer projects. An archive was set up of 750 entries from press and journals for the previous four years. From this research eleven key cases are discussed.
